Record BUG-129 resolved and ERR-095 mitigated after gate-attested controller deployment. Evidence: PR gate 1481, staging gate 1483, one-minute deploy 1484, mutation git-fetch count zero, controller hash validation, exact main/staging/public/state SHA, zero restarts, health/Swiss Ephemeris/401/schema/RLS/policy/ledger/permission checks, and masked SSH secret. Authenticated report generation and browser PDF acceptance remain explicitly pending. Docs only; production unchanged.
