Consult classification runs beside reserve and append (issue #162 , BUG-1499)

Classification only needs the question, history, name and session model, so it now starts in beforeReserve once moderation passes and is awaited after the question is written. Billing is unchanged: smalltalk is still reserved and refunded by complete_consultation_free, and an unused early call (reserve refused) is capped at the classifier's 3 s fail-open.
